About Hammock Homes Supportive Housing

Hammock Homes is a CalAIM-certified housing provider serving Solano and Sonoma Counties. We offer supportive housing services for individuals with complex health and social needs, including recuperative care, post-hospitalization housing, transitional housing, sober living, Enhanced Care Management (ECM), and Day Habilitation. Serving Vallejo, Fairfield, Vacaville, Santa Rosa, and surrounding Solano and Sonoma County communities, we work with Partnership HealthPlan members and accept referrals from hospitals, case managers, and care coordinators. Our homes provide a safe, stable environment where residents receive support to stabilize their health, maintain sobriety, and build independence.
